In my recent article, we explored the "itis" epidemic. Using the pie chart, we mapped out how conditions ranging from Arthritis to Colitis and Bronchitis are all connected. It illustrated a powerful truth: these seemingly different diseases actually share the exact same root cause, which is chronic inflammation, a biological fire that the body cannot seem to put out.

If inflammation is the root cause, how do we extinguish it safely? In my last article, I discussed various ways to reduce inflammation, including food, lifestyle, and natural compounds. Two of the most powerful natural tools we have are the non-intoxicating cannabinoids: Cannabidiol (CBD) and Cannabigerol (CBG).
Today, I am diving in to show why they work and the clinical evidence proving their ability to put out the fires of chronic inflammation.
How Do CBD and CBG Lower Inflammation? (The Mechanism)
To understand how they work, you have to think of the body's inflammatory response like a fire alarm system. When cells are stressed, a master switch called NF-kB is flipped. This is the alarm ringing, telling the body to release "fuel" (pro-inflammatory cytokines like TNF-alpha and IL-6), which causes pain, swelling, and damage.
The CBD Mechanism: CBD acts like a cellular firefighter. It does not get you high. Instead, it interacts with various regulatory receptors in the body (such as TRPV1 and PPAR-gamma) to cut the wire to the NF-kB fire alarm (Buchaim et al., 2025). By doing this, CBD stops the body from pumping out inflammatory fuel, and instead encourages the release of healing, anti-inflammatory signals (like IL-10) to calm the tissue down (McCartney et al., 2020).

The CBG Mechanism: CBG is known as the "mother of all cannabinoids" because it is the original compound the hemp plant makes before converting it into others. CBG lowers inflammation by targeting different receptors (like CB2 and alpha-2 adrenoceptors). Crucially, CBG actively blocks COX-1 and COX-2 enzymes—the exact same inflammatory enzymes that over-the-counter pain pills like ibuprofen target, but it does so naturally without destroying the stomach (Li et al., 2024).

When combined, CBD and CBG work together synergistically, attacking chronic inflammation from multiple different angles at the exact same time.
The Evidence: CBD and CBG in Action
Below are examples of how CBD and CBG help specific inflammatory diseases.
1. Arthritis (Joint Inflammation)
Study 1: CBD (Heineman et al., 2022)
-
Dose: 6.2 mg/mL applied twice a day
-
Study Design: 18 patients with painful thumb basal joint arthritis applied a topical CBD ointment for two weeks in a randomized, double-blind trial.
-
Study Outcome: Patients experienced massive, significant improvements in their pain levels and a great reduction in joint disability, without any negative side effects.
-
Result Interpretation: This proves that CBD is highly effective at absorbing locally to reduce joint pain. This local enrichment of CBD can be complementary with a highly bioavailable CBD formula that delivers this targeted relief efficiently to keep older adults active.
Study 2: CBD & CBG Combo (Anand et al., 2021)
-
Dose: 10 mg/kg of CBD + 10 mg/kg of CBG
-
Study Design: Mice with surgically induced knee osteoarthritis were given the CBD and CBG oil to see if it could stop joint destruction.
-
Study Outcome: Both compounds drastically reduced inflammation and improved the animals' ability to walk. However, only CBG was able to actively protect the physical cartilage from degrading, while increasing the number of healthy, matrix-producing cells in the joint.
-
Result Interpretation: This indicates that if we deliver CBD & CBG together, it doesn't just numb the pain, it may protect the joint's physical structure from deteriorating.
2. Colitis (Gut Inflammation)
Study: CBD & CBG Combo (Godbloe et al., 2025)
-
Dose: 31 mg/kg CBD + 30 mg/kg CBG
-
Study Design: Mice with chemically induced Ulcerative Colitis (severe gut inflammation) were fed the CBD/CBG hemp extract orally (mixed with Nutella) daily.
-
Study Outcome: The oral extract significantly reduced damage to the colon lining, increased protective mucus-producing cells, reduced overall disease severity, and decreased abdominal pain.
-
Result Interpretation: Gut inflammation requires a solution that can safely coat and interact with the digestive tract. A water-compatible beverage additive could be a great delivery system to get these healing compounds directly to the inflamed gut tissue.
3. Bronchitis & Pulmonary Inflammation (Lung Inflammation)
Study: CBD & CBG Combo (Cabrera et al., 2021)
-
Dose: Up to 50 mg/kg
-
Study Design: Guinea pigs were exposed to bacteria to trigger a massive inflammatory response in the lungs, mimicking conditions like COPD or Bronchitis. They were given CBD and CBG in two different formulas: one using standard MCT oil, and one using a micellar (water-compatible) solution.
-
Study Outcome: The standard oil completely failed to reduce lung inflammation. However, the water-compatible formulation successfully dropped inflammatory cell infiltration in the lungs by up to 65%.
-
Result Interpretation: Because traditional cannabinoid oils have poor absorption. When it comes to delivering CBD & CBG effectively to organs that are far away from the digestive system, such as in the lungs, water-compatible administration shows unique advantage due to its quick onset and high bioavailability. How CBD & CBG are delivered plays an important role to achieve consistent clinical outcomes.
4. Steatohepatitis (Inflammatory on fatty liver disease)
Study: CBD and CBG (Kočvarová et al., 2026)
-
Dose: Escalating daily doses of CBD (5 up to 10 mg/kg) or CBG (12.5 up to 25 mg/kg).
-
Study Design: Mice were fed a high-fat diet for 14 weeks to induce severe obesity, metabolic dysfunction, and steatotic liver disease (MASLD/MASH). They were then given daily treatments of either CBD, CBG, or a placebo for 4 weeks to see if the cannabinoids could reverse the damage.
-
Study Outcome: Both CBD and CBG significantly improved glycemic control, normalized fasting blood glucose, and drastically reduced toxic triglycerides and cholesterol in the liver. Mechanistically, the cannabinoids put out the metabolic fire by restoring the liver's lysosomes (the cellular waste-disposal centers) to clear out inflammatory lipids called ceramides, while simultaneously triggering the liver to store "phosphocreatine" to create a massive energy buffer against future stress.
-
Result Interpretation: Metabolic liver inflammation is driven by toxic lipid buildup and cellular exhaustion. This study proves that CBD and CBG provide a completely unique, receptor-independent mechanism to heal the liver's natural waste disposal system. When it comes to delivering these protective benefits to organs dealing with metabolic syndrome, a highly bioavailable, water-compatible administration could offer a unique advantage. It ensures patients receive the consistent clinical outcomes needed to extinguish metabolic inflammation at its cellular root.
Conclusion: The Future is Real-World Evidence
As we have seen across different forms of the "-itis" epidemic, the science is clear: both CBD and CBG possess profound, scientifically validated mechanisms to reduce inflammation at the cellular level. More importantly, because CBD and CBG target the inflammatory "fire" through different biological pathways, combining them creates a powerful, complementary synergy that tackles systemic inflammation from multiple angles simultaneously.
While the preclinical models and early clinical trials are incredibly promising, there is still a gap we must bridge. To truly legitimize this science, we need more human observational studies. We need to move beyond the petri dish and understand the real-world impact: How are these compounds improving patient-reported outcomes? How are they restoring a patient's overall quality of life?
